Our goal is to provide the best medical care for your children. To do this successfully, we must be able to review all of your medical records from all providers that you have seen. We ask to receive your previous medical records before we establish care at your first visit.

For New Families Transferring To Us

When transferring a child to a new pediatric practice, it's essential to gather and organize all medical records, including vaccination records and any relevant test results. Communicate with both the current and new pediatric practices to ensure a smooth transition of care, providing necessary information about the child's medical history and ongoing treatments.


Our office requests to receive your previous medical records prior to your first appointment. If the entire chart is unavailable in time, we ask that you please have your prior doctors office fax the vaccine records and last well summary to our office in the meantime.


Please be aware that under federal (HIPAA) and state laws, your previous medical provider may charge a "reasonable, cost-based fee" to copy your children's medical records. This cost is your responsibility. This fee may be waived if records are sent directly to our office. If you have not already done so, please sign a release of records form and provide it to your children's previous medical provider.
